    Job Description

    Position Summary

    As a member multi-disciplinary Team, the REGISTERED NURSE practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.

    Home Peritoneal Dialysis Unit

    Home Peritoneal dialysis allows people with end-stage kidney disease to perform their own dialysis treatment at home. A reusable flexible tube is placed in the abdomen and special sugar or starch solutions are infused, retained for a prescribed amount of time then drained. The solutions help rid the body of waste products and water, mimicking the action of the kidneys.

    Dialysis Start Unit

    Support patients newly diagnosed with end-stage kidney disease requiring both dialysis and education about the various types of kidney replacement. Patients stay in the unit for six weeks receiving hemodialysis until they decide which mode of kidney replacement treatment is best for them.

    Our services:

    Provide a minimum of seven days training to educate the patient and family members about home peritoneal dialysis. Operate a twice-weekly multidisciplinary clinic. Support new dialysis patients in decisions about their treatment options. Perform home visits. Liaise with home care and industry partners regarding home nursing and home dialysis supplies. As a member of the UHN and the Nursing Multi-disciplinary Team, the REGISTERED NURSE practices in accordance with relevant legislation/standards, meeting the legal and ethical requirements of the profession. The principal responsibilities include: applying knowledge of nursing science, theory, skill and critical decision making in the provision of compassionate, collaborative and competent care in accordance with the UHN Professional Practice Model for Nursing and the mission and vision of nursing at the UHN; ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and the UHN policy and procedure; demonstrating commitment to professional life-long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession.

    Qualifications

    Current Registration as a Registered Nurse with the College of Nurses of Ontario preferred. Basic Cardiac Life Support ( certification. Minimum one (1) year Medical/Surgical experience. Completion of the Nephrology certificate or CNeph C.

    Must have a minimum five (5) years experience in nephrology in-patient care, peritoneal dialysis and/or hemodialysis.

    Current Peritoneal Dialysis teaching experience mandatory. Certificate in Methods of Adult Education preferred. Excellent organizational and time management skills. Exceptional decision making and problem solving skills. Experience with the Baxter Home choice/Claria machine an advantage. Family training experience with Peritoneal Dialysis. required Strong interpersonal and communication skills. Second language an advantage. Valid Driver's License required.
