    Job Summary:
    The Approaches to Community Wellbeing is a regional resource to 31 First Nations communities. The

    Approaches to Community Wellbeing provides expertise, support, services, and programming in public

    health to the region. The program also supports communities in the development, implementation, and

    monitoring of their own Approaches to Community Wellbeing.

    The Vaccine Preventable Diseases Coordinator will support the coordination of vaccine clinics within

    communities. This will encompass routine immunization clinics, as well as clinics to increase the uptake

    of influenza and COVID-19 vaccines.

    The Vaccine Preventable Diseases Coordinator will be accountable to the Preventing Infectious Diseases

    Manager.


    Qualifications:

    • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
    • Current Certificate of Competence from the College of Nurses in Ontario
    • Baccalaureate Degree in Nursing from an approved University with identifiable community
    health nursing content


    • Minimum of three years' experience in public health, infectious diseases, and immunization
    • Project Management certificate or courses are an asset
    • Valid Ontario G drivers licence
    • Ability to perform the requirements of the role on a regular basis

    Roles and Responsibilities:

    • Work with ACW's health promotion teams to ensure that communities have access to the materials
    they need and are connected with personnel to support them in health promotion efforts


    • Develop presentations and materials for community members, community health workers, and staff
    to promote vaccination


    • Provide guidance, supervision, and support to community-based Vaccine Leads
    • Work with communities in planning clinic operations
    • Maintain a list of vaccine coordinators at the community level, along with other key contacts
    • Advise community on requirements for clinic location and set up, and confirm appropriate
    venue


    • Support communities in requesting Rangers, if required
    • Confirming with nursing stations that the supplies and vaccine have arrived in advance of the
    clinic


    • Ensure ground transportation for the vaccine team and home visits is secured and
    communicated to the team lead


    • Accommodations are secured if required, or a contingency plan is in place if the team is
    weathered in


    • Assign staff roles for clinic operations
    • Answer questions community members have about clinic operations
    • Coordinate, with internal and external partners, an annual influenza campaign including
    • Regional health promotion initiatives
    • Support for community-level campaigns
    • Advertising how individuals can access the influenza vaccine
    • Develop, in collaboration with Raising our Children, Community Wellbeing Nursing, Tribal
    Councils, community Health Directors, and Indigenous Services Canada, a strategy to increase

    uptake of routine childhood immunizations.

    This may include, but not limited to:

    • Clinic strategies
    • Health promotion campaigns
    • Community engagement
    • Work with Roots for Community Wellbeing team to monitor immunization coverage rates
    • Other duties as assigned
