The Social Worker will be part of the Outpatient Mental Health (OPMH) team in the St Joseph's Mental Health & Addictions program.


The Social worker, in conjunction with an interdisciplinary team, will provide comprehensive psychosocial assessments, individual therapy, group therapy, case management and discharge planning in the Outpatient Mental Health Program.

The Psychiatric Day Hospital is a partial hospitalization program for clients of St. Joe's who require mental health stabilization. Clients receive individual and group support from our interdisciplinary staff.

The social worker on the team provides brief psychotherapies, psycho-educational group interventions and case management services. The social worker also provides crisis intervention with Day Hospital clients when required.

The social worker, in collaboration with the Day Hospital Team, will be responsible for the:

  • Coordination and updating of the group schedule
  • Development of groups
  • Liaising with Day Hospital guest group facilitators
This position reports to the Manager, Outpatient Mental Health


Qualifications include:

  • Master of Social Work Degree required
  • Current registration in good standing with OCSWSSW
  • Day Treatment/Day Hospital experience preferred.
  • Minimum 5 years direct experience in acute hospital and/or outpatient mental health setting.
  • Experience providing brief psychotherapies and supportive counseling. Preference will be given to those with experience providing ACT, DBT and CBT.
  • Experience in developing, organizing and facilitating psychoeducational groups.
  • Excellent knowledge of community resources and the ability to develop new connections with external services.
  • Ability to facilitate appropriate transitions to external services in the context of a treatment window.
  • Demonstrated ability to be a positive and productive member of an interdisciplinary team while also being able to work independently.
  • Comfortable working with a broad range of client groups and their families/supports. Second language is an asset.
  • Experience participating in Quality Improvement Initiatives or willingness to do so.
  • Satisfactory attendance record.
