The Mount Sinai Academic Family Health Team – Vaughan site is looking for a Nurse Practitioner to join the team to support us on our journey. In collaboration with the interdisciplinary team, the Nurse Practitioner – Primary Care (prevention) will be responsible for performing clinical duties within their scope of practice, including assessments, diagnosis, treatment, follow up, counseling, screening, education and referral for all patients. This role will also be responsible for implementing a Catch Up – Keep Up program at the Vaughan site. The aim of this program will be to bring patients who have fallen behind in their preventative care schedule as result of the Covid- pandemic up-to-date with preventative screening and/or the management of their chronic conditions.
Duties will be performed at the clinic through scheduled in-person and virtual appointments.
• Registered Nurse in the Extended Class - Specialty certification in Primary Health Care.
• Current registration in good standing with the College of Nurses of Ontario.
• Masters degree in nursing (preferred).
• Eligible for cross appointment to an academic institution (preferred).
• Demonstrated commitment to evidence-based practice and professional growth and development.
• Two years clinical experience in primary care or a relevant ambulatory care environment (preferred).
• Ability to work independently and think critically in a diverse clinical environment.
• Knowledge or experience with preventative care and chronic disease management would be an asset.
• Sound clinical judgment and strong health assessment and diagnostic skills.
• Strong interpersonal skills and leadership abilities as an effective member of an interdisciplinary team.
• Demonstrated active participation in knowledge transfer with students and other professional disciplines.
• Experience with program development and evaluation and/or change management.
• Ability to assist in the development and review of clinical processes and procedures in order to provide high quality and efficient patient care.
• Ability to uphold equity, diversity and inclusion principles within a culturally diverse, community-based context.
• Understanding of, and commitment to, patient and family-centred care.
• Demonstrated satisfactory work performance and attendance history.
