    The Sector Support, Homecare and Community Services team will provide leadership and support for ensuring all patients receive accessible, high-quality homecare and community services.

    With deep content knowledge centred around delivery of homecare and community services, the team will collaborate across OH and with our partners (including Ministry and HCCSS) to provide support for this sector.


    As the Director, Sector Support, Homecare and Community Services within the Sector Support, Performance and Accountability Portfolio, you will be responsible for leading a high-performing team focused on providing support to all aspects of delivering care in the home and community, including community support services and assisted living.


    Here is what you will be doing:
    Provide leadership and support for modernizing homecare delivery in Ontario, including through the Ontario Health team model.


    Provide leadership and expertise in areas of policy, funding, planning, accountability, and support issues and risk management related to homecare and community care.


    Support improvement in quality and transitions in care for patients and caregivers utilizing homecare and community services through implementation and operations of quality improvement initiatives.


    Lead and partner on strategies that support improvement in access and flow within the homecare and community sector, and between other sectors.


    In partnership with OH regional teams and program areas, lead Ontario's Health's Annual Business plan initiatives related to homecare and community care.

    In partnership with HCCSS, co-lead the implementation of the OH-HCCSS joint workplan.

    This includes a mix of operational support for elements related to quality care and education as well as support for key initiatives and processes such as leading elements of projects related to medical equipment and supplies, service provider pre-qualification processes etc.

    Support provincial collaboration and partnership associated with the homecare and community sectors, by establish a sector support and partnership model with partners and providers providing care in home and community sectors, including establishing and leading governance and engagement tables.


    Support implementation of recommendations to drive improvements in the home and community sectors including through implementation of policies and Auditor General recommendations.

    Uses strong communication and negotiation skills to develop consensus around complex policy and/or program related sensitive or contentious issues.


    Manages the program and its staff to support an environment and culture of service excellence which respects diversity, encourages all employees to work together to achieve results and contribute to a healthy, rewarding, and productive working environment where personal and team accountability are key to meeting the mandate of OH.

    Accountable for building a strong team including hiring, orienting, and training of staff, determination of performance standards and conduct of performance reviews, and managing employee relations matters.

    Here is what you will need to be successful:

    Education and Experience

    Experience working within or partnering with homecare and/or community services is a must

    Post-Secondary degree or equivalent in business or health administration, public policy, health policy, or clinical profession (nursing, social work, allied health, pharmacy, etc.) required

    Graduate/Master's degree or equivalent experience in applicable field (business or health administration, clinical profession (nursing, allied health, pharm, etc.) required

    Minimum of 10 years of experience working with guiding, influencing, implementing, etc. policy at system level (can be done as provider contributing frequently to provincial sector tables), with a minimum of 10 years in progressive leadership positions

    Experience leading multidisciplinary and cross-organization teams with formal and informal lines of accountability/reporting structures

    Experience in forging functional partnerships with Ministry and provider associations

    Experience and demonstrated success working with senior/executive level teams and clinical leadership

    Proven team leadership and management experience, with ability to recruit, engage and develop staff with advanced degrees/knowledge and diverse backgrounds

    Develop emerging leaders who are able to build strategic partnerships with internal and external stakeholders to move departmental and portfolio mandate


    Work collaboratively with Regional teams and Analytic teams to provide knowledge and advice based on data, to reduce barriers, monitor health system performance and develop insightful forecasts and opportunities to improve care in the homecare and community care sector.

    Knowledge and Skills


    Experience and knowledge of provincial home and community services policy, legislation, and regulation, and the challenges and opportunities for homecare and community care in Ontario or other jurisdictions.

    Demonstrated people leadership skills, including ability to coach and mentor team members

    Demonstrated ability to successfully develop and implement strategies, programs, and initiatives


    Demonstrated experience communicating with Senior Leaders including presenting to Executive Teams and Board Committees, and communicating with partners including the Ministry of Health.

    Strong skills, with demonstrated experience engaging and building relationships and consensus among diverse stakeholders

    Demonstrated ability to successfully execute on multiple large-scale, complex, high-risk projects, ideally within a cross-functional structure

    Excellent verbal and written communication skills, including the ability to present to large groups, prepare briefing notes, reports, policy papers and evidence summaries

    Demonstrated experience building relationships with a range of system stakeholders and diverse interest groups

    Ability to lead through ambiguity and complexity

    Additional roles & responsibilities may evolve and change based on need of stakeholders and the portfolio
