Daily coordination with midwives, obstetricians, family medicine, paediatrics and nursing to identify and coordinate early discharge planning as soon after birth as possible. Providing home visits for family post discharge, ensuring safe, high quality follow up for hospital, clinic, or office visits, for newborn screening, jaundice screening and CCHD screening. To provide gaps in service needs to our partners and patients on the obstetrical unit, for example stepping into the primary care role, supportive care in labour, assisting at codes, assisting at caesarean sections, assisting with postpartum discharge planning and teaching.Requirements
