Position Summary
The Quality Improvement Lead operates at an operations management level, fostering collaborative partnerships with operational managers within Mosaic Primary Care Network (MPCN).
This pivotal role involves joint analysis of data to proactively uncover opportunities for refining both clinical and non-clinical programs.
By closely engaging stakeholders, the role drives the co-creation, execution, and oversight of improvement initiatives geared towards enhancing operational efficiency and quality standards—ranging from acceptability and accessibility to appropriateness, effectiveness, efficiency, and safety—across MPCN programs.
This collaborative effort aligns seamlessly with the objectives of Mosaic PCN, Calgary Zone, and Provincial targets.
The QI Lead will collaborate with cross-functional teams, including clinical and Non-clinical managers and their staff, to leverage best practice, enhance and optimize processes, identify unwanted variance, and support compliance with program standards, and support continuous quality improvement.
This involves the identification and rectification of deviations, upholding compliance with program benchmarks, and nurturing the culture of continuous quality advancement.
This role is committed to integrating quality metrics, patient-reported experience measures (PREMs), and patient-reported outcome measures (PROMs) to assess program efficacy and amplify patient contentment, thereby cultivating a comprehensive culture of quality improvement.
This role will coach clinical and non-clinical managers and evaluation team members regarding QI process, and coordinate for all components of quality improvement practice including: planning, designing, implementing, monitoring, adapting, and sustaining.
Key Responsibilities
Strategic Collaboration for Clinical Programs:
- Engage proactively with clinical programs at Mosaic Primary Care Network (MPCN), actively contributing to process development, implementation, and the continuous refinement of practices.
- Partner closely with program stakeholders to align the aspirations and objectives of the program with MPCN's overarching mission and vision.
- Contribute actively to program planning, evaluation, and the tracking of performance, fostering a collaborative approach to improvement.
Empowering the Medical Home Support Program:
- Play a key role in supporting the seamless implementation and ongoing enhancement of the Medical Home Support Program within MPCN.
- Collaborate effectively with the REDI Manager, Program Manager Medical Home Support, and program stakeholders to ensure program adherence, compliance with MPCN's strategic direction, and alignment with Zone and Provincial guidelines.
- Utilize data analysis, encompassing quality metrics, PREMs, and PROMs, to assess and elevate the patient care experience within the framework of the Medical Home model.
Insightful Data Analysis and Communication:
- Gather, analyze, and interpret programspecific data, including quality metrics, PREMs, and PROMs, to pinpoint areas for improvement and systematically track program outcomes.
- Identify data sources, design data collection plan and analytical plan, deploy analytics tools and methodologies adeptly to evaluate program efficacy, patient experiences, and clinical outcomes.
- Perform advanced analytical techniques include statistical modeling, marching learning, and artificial intelligence, effectively translate analytical concepts into plain language for nontechnical stakeholders.
- Collaborate closely with the Reporting, Evaluation, and Data Integrations (REDI) team, as well as the Clinical Informatics Analyst role, to develop and maintain data dashboards and comprehensive reports that effectively communicate insights to MPCN stakeholders.
