The Opportunity:

We are recruiting a
MENTAL HEALTH CLINICIAN (Seniors Services)

  • This is a contract fulltime opportunity (35 hours per week Monday to Friday 9am to 5pm for approximately one year with the possibility of extension) based at New Vision Family Health Team, 421 Greenbrook Drive, Kitchener.


As a Mental Health Clinician who is person-centred, team-oriented and collaborative in your approach, you will work together with older adults and their care partners living with chronic disease, complex mental health and/or addiction issues and/or dementia, to optimize functioning and quality of life.

The Mental Health Clinician will provide interventions utilizing findings from comprehensive mental health assessment and consultation. Demonstrated ability to assist with risk mitigation and advocacy is a must.

The Mental Health Clinician works closely with an integrated care team inclusive of Geriatric Psychiatry, Nurse Practitioners, Home and Community Care Support Services, Intensive Geriatric Service Workers and Primary Care.

This position reports to the Manager, Seniors Services (CMHA WW).


Qualifications:

  • Masters level in social work
  • Registration with relevant college/registering body.
  • Experience and understanding of how concurrent disorders impact the lives of individuals is an asset.
  • 35 years of experience in the provision of services to an older adult population with medical, cognitive, mental health, addictions, and behavioural complexities.
  • Skills, knowledge, and clinical practice commensurate with the administration and interpretation of geriatric assessment, consultation, care planning implementation and intervention functions.
  • Sound understanding and demonstrated knowledge of the Substitute Decisions Act and Health Care Consent Act.
  • Sound working knowledge of Ontario privacy laws as an enabler to safe quality care.
  • Demonstrated ability to flag issues of capacity.
  • Ability to adapt to rapidly changing risk situations.
  • Welldeveloped assessment and intervention skills specific to an older adult population with conditions such as dementia, mental health issues, agerelated issues, and issues related to family dynamics.
  • Ability to provide education to colleagues regarding seniors' mental health issues in general and related to older adult patients seen in practice.
  • Demonstrated understanding and sensitivity to social and cultural diversity.
  • Highly developed organizational, communication and problemsolving skills.
  • The ability to work collaboratively in a shared care model with colleagues from multiple health and social care disciplines.
  • Valid driver's license, vehicle, and insurance, as travel is required.

Level 6 (OPSEU) - $34.28 to $41.78 per hour
