Community and Partnership Coordinator

Hours of Work: 37.5 Hours per week

ABOUT ST. CLARE'S

St. Clare's is a non-profit affordable housing provider that owns and operates seven apartment buildings in Toronto's downtown core.

Our tenants are referred to our buildings through various partner agencies in the City of Toronto and support for them is provided by their referring agency.

Many of our tenants have experienced homelessness and manage challenges with mental health, addiction, and trauma.


THE OPPORTUNITY


We are currently looking for a Community Partnership Coordinator (CPC) to coordinate support for tenants with their referring agencies in one of our larger rental properties.

The CPC works in collaboration with the Property Manager, Tenants, and Referring Agencies to achieve successful tenancies.

ACCOUNTABILITY
Reports to the Director, Community and Partnership


FUNCTION
The Community and Partnership Coordinator works in collaboration with the Property Manager, Tenants, and Referring Agencies to support St.

Clare's Tenancy Model, "Successful Tenancies - Successful Communities" and to facilitate and maintain healthy partner relationships with our Referring Agencies.

The CPC collaborates with partners and within St. Clare's Community Housing Partnership's procedures and policies to promote social inclusion and prevent eviction.

The CPC provides crisis counseling, brief therapy intervention, uses facilitation and mediation skills, supports any necessary referrals, and ensures barriers are removed to promote the success of tenants.


DUTIES

  • Tenants_
  • Establish and maintain a supportive network with referring agencies and St. Clare's staff that will assist the tenants to maintain a successful tenancy
  • Use brief therapy and mediation skills, knowledge of community networks and resources to support skills development, promote social inclusion, and prevent eviction
  • Assist tenants in attaining community outcomes and goals
  • Assist tenants in reviewing their personal goals with their partner agency and reevaluating the outcomes on a regular basis
  • Promote open communication between all parties involved in the building and partnerships
  • Proactively involve tenants in the discussions and decisions that affect them
  • Provide information and referrals to tenants, (and coaching if needed,) regarding available services and community resources; support them to access and evaluate them
  • Support the tenant to advocate for appropriate resources as needed
  • Coordinate community orientation sessions and workshops for tenants; and facilitate monthly tenant meetings to address building and community issues
  • Support tenants who currently do not have a referring agency

  • Provide leadership to the development and implementation of the St. Clare's Community Partnership Model
  • Work in collaboration with partner agencies in the referral, support planning, and movein process
  • Work with referring agencies to develop and maintain a transparent and agreed upon prioritizing process for vacancies
  • Liaise with other service providers involved with each tenant and support tenants to work effectively with their support service providers
  • Work with other Partner Agency staff and onsite staff as part of the treatment and support team for tenants as needed
  • Work with the referring agencies in scheduling and supporting delivery of workshops, training sessions, and community development initiatives that involve the tenants
  • Work collaboratively with partner agencies to develop and implement strategies to prevent evictions and to create discharge plans if those strategies are not successful
  • Encourage good coordination of resources and strategies among participating service providers
  • Work in collaboration with the partnership and tenants to evaluate outcomes of St. Clare's Community Housing model
  • Administrative_
  • Collect and maintain statistical information on tenants moving in and out
  • Develop and administer surveys and questionnaires for all tenants moving in and document the outcomes of tenants leaving, and periodically during tenancy
  • Regularly report activities for consultation and/or direction to his/her peer team and Director on a regular basis (every two weeks or more often if necessary)
  • Attend meetings, case consultation meetings and staff training and development opportunities
  • Prepare case records, statistical report and other administrative tasks as required

Requirements:


  • Master's degree in social work, psychology, counselling or equivalent education and experience.
  • 5 years experience working with clients in a facetoface setting
  • Experienced in brief counselling and being a catalyst for change
  • Knowledge of addictions
