Program Description:

At Surrey Place, we value the collaboration involved in connecting our clients with the help they need.

Our Intake team acts as the front door to our organization through welcoming clients, caregivers, service providers and other staff, and then connecting them to the resources they are looking for.

Using a collaborative approach, we provide service coordination consultation, referrals, and information about other community and funding supports, to find the most appropriate services for each individual client.

Focusing on our client as the priority, we collect information and develop individualized services plans to help our clients achieve their goals and live their full potential.


As part of the Canada Summer Jobs Program, the Children's Intake and Wellness program is seeking a Service Navigator/Intake Assistant.

This position reports to the Manager, Intake Cross Agency Clinical Services.


Please note that as this position is a part of the Canada Summer Jobs Program, it is therefore dependent on funding

Main responsibilities:


The Intake Service Navigation Assistant is responsible for supporting our Intake Workers and Service Coordinators as they process and direct new clients to the appropriate clinical services.

This role has a clinical administration and data focus.

The duties include, but are not limited to:

  • Respond to service inquiries via telephone and confirming client eligibility.
  • Complete client registration.
  • Data entry onto Microsoft CRM for medical services and database registration for coordinated service planning.
  • Assemble and send out welcome packages to new clients.
  • Project work with the Manager, Service Coordination & Intake and with Service Coordinators.
  • Support running service clinics virtually.
  • Photocopy documents for client and departmental distribution.
  • Retrieve faxes and distributing to staff.

  • Excellent communication, teamwork, and problemsolving skills
  • Newcomers / refugee youth (Newcomers are defined as having arrived in Canada within the past 5 years)
  • Indigenous youth
  • Visible minorities / racialized youth
  • LGBTQ2 youth
  • Youth with disabilities
  • Youth who have not previously been employed and for whom this would be their first job experience.
