DUTIES:
The Incumbent will work under the supervision of the Manager of the Day and Stay Program/Designate by providing direct and indirect care as required in collaboration with other health care workers
Duties will include:
performing personal care, observing and reporting changes in client physical and emotional conditions, dealing tactfully and courteously with clients, community partners, and families.
The incumbent must be able to provide risk management and IPAC measures. The position may include weekends. The incumbent will be required to perform on-call duties. This is a 7.5-hour shift.QUALIFICATIONS:
Graduate of an approved Registered Practical Nurse Program OR Social Worker/Social Service Program OR Therapeutic Recreation Program
Hold a current College of Nurses of Ontario certificate in Competency OR hold a registration with Social Worker/Social Service Worker College
Demonstrate satisfactory experience as an RPN, TR or SW/SSW
Good physical and mental health
Demonstrate strong positive personal and interpersonal communication skills, both verbally and written. Ability to work as a member of a multi-disciplinary team.
