The Relief Mental Health Worker (Relief MHW), rural services will support participants accessing services by providing a psychosocial response in order to promote crisis resolution and empowerment at their time of need.

The Relief MHW will also work collaboratively with participants to assess and identify their goals and link them to appropriate supports in the community.


Depending on the program, supports may be provided in-person, via telephone or on a walk-in basis at the Strathroy site; at Strathroy Middlesex General Hospital or in the community.

WHAT WE'RE OFFERING:

What you'll be making - $ $25.71 per hour, rate based on proof of comparable, previous experience; mileage between sites reimbursed per kilometre

When you'll work:

  • 14 hours over a twoweek period; casual shifts Monday-Friday 8:30am4:30pm

HOW YOU WILL BE MAKING A DIFFERENCE:

_ Crisis Assessment-In collaboration with the participant:_

  • Complete facetoface mental health assessments based on theory of crisis management including rapid response, risk assessment, deescalation, stabilization, and referral to determine participant needs and level of risk
  • Establish a therapeutic relationship to maximize the benefits of engagement between the staff person and the participant
  • Engage Emergency Services when it is determined that additional intervention is required
  • Assess social and environmental factors impacting the participant
  • Assess the adequacy of resources/supports currently available to the participant in the county
  • Determine the impact of psychiatric symptoms on functioning
  • Identify risk factors related to the safety of participants and staff as well as relapse
  • Assess barriers to successful community living (e.g. treatment options, transportation, finances, etc.)
  • Ensure the assessment incorporates cultural and ethnic factors
  • Obtain information, as needed, from other relevant sources
  • Identify areas requiring additional assessment

_ Case Management and

Community Wellness:
_


  • Advocate as necessary in order to facilitate access to services and supports with the objective of remediating immediate crisis situation
  • Support the participant to access needed resources
  • Assist the participant to identify and connect with personal/natural supports
  • Provide shortterm followup support to bridge the gap from referral to engagement with a longerterm support service
  • Provide, facilitate and model an atmosphere of mutual caring and trust among participants in Community Wellness programs by building relationships with participants and responding to their needs
  • Monitor, assess and provide feedback regarding participants progress both to the participants and members of their support team to assist in the participants' goal achievement
  • Provide transportation to participants as required to facilitate participant in Community Wellness or other service needs
  • Refer participants to a Crisis Assessment Team (CAT) member when psychiatric assessment for a clinic referral is determined to be necessary and/or appropriate

_ Service Coordination and Advocacy:
_


  • Have knowledge about rural community resources relevant to participants, including other mental health services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, Middlesex Housing, etc.)
  • Identify, develop and maintain good links with a wide range of community resources and community partners
  • Assist the participant to obtain needed resources by educating and supporting the participant to access resources, linking the participant to appropriate services, advocating with or for the participant when needed
  • Demonstrate knowledge of community resources and its place within the mental health care system
  • Coordinate and facilitate, with participant, linkages to recommended services and supports in the community
  • When appropriate, act as liaison with caregivers and service/support providers who are involved with participant to facilitate a coordinated approach to service provision
  • Assist in building positive working relationships across the service system
  • Participate in public education and advocacy

_ Information and Referral:
_


  • Serve as initial contact in greeting and directing participants, staff and guests to CMHA Strathroy site with a primary focus on providing a welcoming environment to all site visitors
  • Provide telephone assistance to participants seeking support, facetoface appointments and/or information about mental health services & supports
  • Provide health teaching and information regarding a variety of coping techniques and strateg
