    We are seeking:

    Care Team Scheduling Coordinator (Integrated Client and Community Care)

    Permanent Part-Time Position, Flex Shifts

    Guaranteed 20hrs per week, required Monday-Sunday availability

    The incumbent must be available to attend a 3-week full-time in-person training, starting January 29th, 2024 , including weekdays and weekends, from 8:30am-4:30pm.

    Shift information: shifts will be scheduled any day of the week, including weekends and STAT holidays. Hours of work will range from 8:30am-9:30pm (Monday to Friday) and 6:30am-9:30pm (weekends and STAT Holidays).

    Successful candidates must be available for rotating shifts during those times, any day of the week between Monday through Sunday, including STAT holidays.

    What is the job?

    Reporting to the Integrated Operation Supervisor, you will help our clients maintain their independence and brighten their day by fielding client calls and ensuring their services are scheduled in a timely manner. You'll work in a fast-paced high energy customer service center, communicating with clients, families, funders and clients' Health Care Teams including Personal Support Workers, Nurses and Rehab Service Providers. You'll be supported by a passionate team and state- of-the art technology that enables you to coordinate services, accept new client referrals, schedule service changes and maintain client service plans.

    Am I qualified?

  • The ideal candidate has completed Post-Secondary Education (in healthcare preferred) and has related work experience in a direct client service role or an environment where staff and clients are your number one priority
  • Ideally you also have experience working in a healthcare organization/agency or in an administrative position that required extensive scheduling/multitasking
  • Excellent communication skills that demonstrate empathy, compassion and a client and a family-centered approach are essential to this role
  • Solid time management skills, attention to detail and the ability to prioritize tasks are equally important
  • You must be able to work independently while being flexible enough to support the team as changes arise
  • Medical terminology would be an asset
  • Satisfactory completion of the technical skills testing is required.
