Program Overview - Supportive Housing, Community Care
The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues.

The program provides onsite psychosocial, primary care supports and services including:

intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.


What You Will Do

  • Conduct tenant needs assessments and create care plans for individuals who have current or past hoarding tendencies and /or limitations for unit maintenance. Engage tenant in identifying any issues/problem areas inside the unit for creating a healthy living environment.
  • Encourage and support tenant engagement in the ongoing management of their unit to achieve positive outcomes related to decluttering, garbage removal and preparation for pest treatments. Collaborate with Housing team and external contracts for the disposal of items.
  • Coach and educate tenants on their responsibilities as per the signed tenancy agreement with WoodGreen. Work with tenants to ensure a clear understanding of housing concerns as related to fire safety with hoarding and participation in pest control treatments.
  • Identify resources and develop collaborative plans with tenants in respect to issues of hoarding behaviour.
  • Work with and educate tenants to understand the relevance of pest control management and support tenants to properly prepare units for pest control treatments.
  • Ensure tenant ownership and empowerment through processes of decluttering units.
  • Adapt and evaluate decluttering plans and programs on a regular basis while working with tenants to meet changing needs.
  • Collaborate with support teams, Supervisor/Manager, and housing staff to plan and facilitate appropriate pest control treatments.
  • Help tenants to set and achieve realistic goals with respect to bringing their units to an acceptable safety standard. Help tenants to achieve set goals through supportive means such as encouragement, coaching, psychosocio education, and deescalation.
  • Attend Hoarding Network and other communityrelated meetings to network and keep up to date on best practices for hoarding supports and follow up for behavioral issues that impede progress.
  • Liaise with Supervisor, Manager, and support staff team to coordinate information sessions for tenant communities and 'onetoone' to coaching on decluttering and maintaining healthy living environments.
  • Provide crisis intervention and conflict resolution with residents when required.
  • Monitor and review tenant needs and community issues and communicate the information to the Supervisor of Housing Supports and/or Manager.
General Responsibilities

  • Carry out administrative procedures and maintain appropriate records with respect to pestcontrol treatments and interventions.
  • Uphold confidentiality policies regarding tenant information.
  • Prepare statistical and narrative reports/updates of the interventions for the Manager.
  • Responsible for upgrading training and education as directed.
  • Ensure the antiracism objectives and principles of the organization are achieved within the program.
  • Represent the unit on internal staff committees as directed.
  • Participate in regular supervisory sessions with the Supervisor and regular staff/unit meetings.

What You Bring to the Team

  • Post-Secondary Degree in Social Work or related field; or Diploma in Social Services with several years of related experience.
  • 2+ years of experience in social services working with individuals who have history of homelessness, low income, addictions and mental health challenges.
  • Knowledge of bed bug and other pest concerns, pest control procedures and solutions.
  • Knowledge of best practices for hoarding support and follow up.
  • Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption).
