    About the Opportunity

    As the Primary Health Care (PHC) Coordinator reporting to the Primary Health Care Manager and Lead, you will serve as a Practice Facilitator, supporting practice and advancing strategic priorities in Primary Health Care, Chronic Disease Management, Wellness, and the Department of Family Practice portfolio. Your responsibilities include planning, developing, implementing, and evaluating services and projects in coordination with primary healthcare teams and stakeholders.

    Collaborating with healthcare staff and external partners, you will drive change management strategies in primary health care services. Your role involves fostering strong relationships with key stakeholders and contributing to the improvement of PHC projects/initiatives.

    About You

    If you meet the following criteria, we want to hear from you:

    • Baccalaureate degree in a health-related discipline
    • Current registration with a professional association
    • Minimum of two years' related experience
    • Organizational and project management skills
    • Chronic Disease Management experience is a plus
    • Strong communication and interpersonal skills
    • Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
    • Ability to work independently and in a team environment
    • Valid Nova Scotia Driver's License

    Hours

    • Permanent, Full-time position; 75 hours bi-weekly
    • Shifts may include evenings, weekends, and holidays

    Compensation and Incentives

    $34.11- $42.68 Hourly

    Successful candidates may be eligible for a benefits package that includes health, dental, travel, long-term disability, life insurance, and a defined benefit pension plan.
