    Description

    Job Title: Social Worker at Extendicare Hillcrest



    Extendicare Hillcrest, a Long-Term Care Home in NW Calgary, Alberta, is seeking a dedicated full-time Social Worker to join their team.


    RESPONSIBILITIES
    • Participate in resident admission process, transfers, and discharge.
    • Advocate for residents' rights and liaison with families.
    • Collaborate in resident assessment and care planning.
    • Provide counseling for residents and families, and evaluate effectiveness.
    • Collaborate with Psychogeriatric Consultant to support staff.
    • Adhere to department plans, policies, and quality assurance programme.
    • Provide in-service programs for staff, residents, and families.
    • Liaise with educational institutions and supervise social work students.
    • Maintain required records, reports, and documentation.
    • Participate in committees related to resident care.
    • Liaise with community partners, company resources, and professionals.
    • Assist in obtaining benefits for residents from Government departments.
    • Other duties as assigned.

    QUALIFICATIONS
    • University Graduate in Social Work, Mandatory.
    • Registration with the Alberta College of Social Workers, Mandatory.
    • Previous experience in a long-term care setting preferred.
    • Excellent communicator, well organized, team player with solid skills.
    • Expertise in working with Senior populations with various psychogeriatric issues, preferred.
    • Knowledge of Long Term Care legislation and standards, preferred.
    • Knowledge of occupational health and safety practices, principles, and legislation, preferred.
    • Computer literate with understanding of RAI-MDS and PointClickCare software system, preferred.
