Reporting to the Operations Manager, the Charge Nurse (CN) assists in facilitating the activities of the inpatient clinical team to ensure compassionate, effective and efficient delivery of high quality care for patients. The CN is responsible for frequent communication with team members throughout their shift and efficiently assigning tasks when needed. Their role entails promoting and fostering a patient/family centered approach within the context of a team-based model of care. The CN supports the ongoing assessment and monitoring of patients health care needs and the development of plans to meet those needs. The CN supports the application of evidence based practice in the delivery of care consistent with the organization's mission, vision and values.
Qualifications:
Qualification/Responsibilities:
Coordination and assisting in facilitating the activities of the patient care team within their respective units and ensuring patient care needs are being addressed and that care delivery is completed in a timely and coordinated manner
· Assists team in the completion of admissions/assessments on patients to determine care plan, predictability and complexity of the patient
· Responsible for clinical aspects of the MDS Assessments on admission and every 90 days per Ministry of Health guidelines with assistance from Health Information Management (HIM)
· Alongside the team will assist in delegating activities to the team based on assessment of patient care needs relevant to scope of practice
· Assigns groups of patients based on patients' needs, complexity and team competency
· Huddles regularly with team throughout shift to ensure team is supported and offering assistance when needed
· Facilitate communication with physicians and other members of the care team on all shifts
· Performing all duties requiring an RN scope of practice and support all other nursing tasks as required
· Supporting shift to shift report amongst the team in collaboration with oncoming CN
· Escalation of any patient care related and staffing concerns to the Operation Manager or Manager on Call after attempting to problem solve
· Takes a leadership role in creatively and critically solving problems with the team to overcome difficult situations
Education and/or Certification:
· Bachelor's degree in Nursing – Preferred
· Specialty Certification in Gerontology, Rehabilitation or Palliative – Preferred
