    Temporary full time (12 months)

    The PATH Program is a community program designed to fa cilitate a smooth transition from hospital to home and ensures that community support services are in place for clients.

    Participate in client discharge planning (hospitals/community support services).
    Ensure staff follow safety protocols and policies of the agency and stakeholders.

    Follow up with PATH clients within 48 hours after service delivery to complete the service, assess for additional services, refer to other services as appropriate , and conduct client satisfaction surveys.

    Assist with preparing and monitoring program specific budgets, complete program statistical reports and assist with preparing progress reports.

    Establish the qualitative and quantitative data required to evaluate the program in consultation with hospital, HCC, and participating agencies.

    A Bachelor's degree or one to three years job related experience or/and an equivalent combination of education and experience.

    Fluency in English is required , French is an asset
    Valid drivers license and access to a reliable vehicle with the ability to travel in the community.
    Proficiency in Microsoft Office applications (Word, Outlook, Excel, Power Point) is required .
    Experience with administrative tasks such as reporting, doc umentation, scheduling calls .
    Standard First Aid and CPR-C training is an asset
    K nowledge of community support services

    The majority of the work will be performed in a clean and comfortable office environment, however travel throughout the assigned geography may also be required .
