- Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of our specialized palliative care team
- Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes
- Continually assess, anticipate, identify and respond to new and changing needs of our patients
- Travel between patients' homes within a specific geographical region
- Minimum of two-year nursing experience (Community Nursing experience is ideal, but not required)
- Currently registered and in good standing with the College of Nurses of Ontario (CNO)
- Must have experience working with palliative care patients
- Fundamentals of palliative care, LEAP and/or CAPSE courses are an asset
- Ability to work independently in managing your own caseload
- Proficiency in English, both verbal and written required (if you can speak additional languages, terrific)
- Valid Ontario driver's license and reliable vehicle with willingness to travel
- Excellent patient assessment and teaching skills
- Problem-solving and critical thinking skills
- Physical ability to perform your day-to-day duties
- Basic proficiency in Word, Excel and ability to learn software
- Flexibility in scheduling – create a schedule that works for you and your work life balance
- Primary Care Model – manage your own caseload and make a difference in patient's lives
- Professional development – continue to grow your clinical skills through education support and training
