    Description
    We are looking for a Part-Time Registered Nurse (RN) to join our Wound Ostomy Continence (WOC) Team in Toronto's Downtown Core.
    • Provide specialized holistic assessment and management as part of the interprofessional WOC Team.
    • Collaborate with the team to formulate treatment recommendations and develop customized care plans.
    • Evaluate care plans, involving specialized nurses in the process as needed.
    • Offer education and mentorship opportunities to enhance wound, ostomy, and continence knowledge within our organization.
    • Consult with care providers internally to ensure optimal service delivery.
    • Support quality healthcare through teaching, mentoring, and program development.
    • Optimize client outcomes by reviewing Quality Improvement reports.
    • Stay updated on nursing trends and issues in specialty wound, ostomy, and continence management.
    • Contribute to research projects and collaborate with professional nursing associations.
    • Participate in local initiatives and working groups related to Home and Community Care Support Services.
    • Requirements include completion of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program, flexibility, commitment to advancing care, ongoing education, and a willingness to travel.
    • Comprehensive knowledge of community healthcare and computer skills are preferred.
