    Description

    Transition Liaison Registered Nurse Job Opportunity at Interior Health

    The Transition Liaison Registered Nurse provides nursing care to patients as part of an interdisciplinary team, focusing on enhancing quality of life and collaborating with healthcare providers. Responsibilities include continuous improvement in nursing practice, coordination of patient access and care transition activities, and communication between different healthcare entities.

    • Conducts comprehensive patient assessments

    This is a casual opportunity at Interior Health, where hours are not guaranteed but can lead to permanent or temporary positions. The hourly wage for this position will be between $41.42 to $55.91, effective April 1, 2024.
