Full Time 1.0FTE (37.5 hours/week) - 2 positions

The EOPCN is recruiting for a Mental Health Navigator

Reporting to the Clinical Supervisor-Mental Health Navigation & Community Navigation, the Social Worker

  • Mental Health Navigator is a blended role in the EOPCN Mental Health and Social work umbrella, supporting
referrals from our member physicians for patients attached to our PCN. As a Navigator, the Social Worker is

responsible for connecting patients to the right provider at the right time through screening, assessment, and

following referral pathways to ensure patients are connected to the most appropriate service. This role also

supports patients who require social work support for finances, employment and housing and requires the

Social Worker to work to full scope, in accordance with the Alberta College of Social Workers Practice

Standards & Code of Ethics, and to maintain current knowledge of resources in the community. This position

works collaboratively with an interdisciplinary team of physicians, nurses, dietitians, pharmacists,

kinesiologists, behavioural health consultants, mental health therapists and psychiatrists to provide patient

support.

KEY RESPONSIBILITIES & DUTIES

  • Interview patients, their families, and other concerned parties to obtain information about personal,
social, and environmental history.

  • Work with patients and their care partners to achieve goals as mutually decided upon by the patient
and their care team.

  • Assess the social determinants of health and develop care plans based on culturally appropriate,
evidence-based interventions.

  • Participate in early detection of emerging patient emotional and behavioural disorders.
  • Cofacilitate patient education workshops to promote biopsychosocial wellbeing.
  • Connect patients to programs and services both internal and external to the PCN. Evaluate the
effectiveness of interventions and adjust care plans accordingly.

  • Advocate to meet patient and family needs such as food, housing, transport, and financial support.
  • Participate in multidisciplinary case conferences and provide consultation and education to support
other health care service providers.

  • Communicate to family physician the status of patient progress through documentation of relevant
clinical information, referral correspondence, patient interaction, and patient care plans in a timely

manner.

  • Develop positive working relationships/partnerships with other disciplines, agencies, and health care
professionals to promote coordinated service, quality care and access to suitable resources for clients.

  • Maintain a high level of current knowledge, trends, and resources in the zone/province.
  • Collect and maintain data for individual clients and program evaluation.

QUALIFICATIONS:

  • Active full registration with the Alberta College of Social Workers (nonprovisional).
  • Diploma of Social Work or Bachelor's Degree in Social Work.
  • 1 year of clinical experience preferred with preference given to those with primary care experience.
  • Knowledge of clinical social work treatment methodologies with emphasis on interdisciplinary team
approaches.

  • CPR level C and Non-Violent Crisis Intervention training is an asset.
  • Ability to deal effectively with patients with mental health concerns who are experiencing crises.
  • Understands and follows applicable legislation (e.g., Mental Health Act, Child Welfare Act, Health
Information Act).

  • Working knowledge of local and provincial mental health resources.
  • Proficient knowledge of Electronic Medical Record documentation and Office 365 Suite.
  • Ability to communicate effectively orally, electronically or in writing.
  • Successful applicants will require a clear Police Information Check with Vulnerable Sector check.
