The Health Information Reviewer is responsible for abstracting key clinical information to determine eligibility for donation from deceased patient charts for hospital and public reporting performance data.

This data is used for strategic planning and to spur donation improvement in the Ontario System

Here is what you will be doing:

  • Select patient encounters for chart review based on identified criteria
  • Organize review schedules in conjunction with partner hospitals, respecting hospital health information resources while meeting TGLN deadlines
  • Travel to hospitals as required
  • Review patient encounter to extract information relevant to determining potential for organ and tissue donation and document according to TGLN standards
  • Follow-up with Organ and Tissue Donation Coordinators (OTDCs) for any clarification required in order to ensure accuracy of transcription of information into database
  • Enter extracted information into TGLN database
  • Request reports from Public Reporting Specialist (PRS) or IT
  • Review mortality lists submitted by hospitals and cross reference with TGLN reports to isolate charts with organ donation potential to identify which charts are required to be reviewed and information abstracted
  • Contact identified Medical Records lead in assigned facility to communicate charts needed for review
  • Transport information in a secure manner
  • Use technology remotely and remain available to TGLN office/other stakeholders as needed
  • Coordinate the most efficient and cost-effective routes to enable all hospitals reviews meet timelines
  • Learn specific charting system of each assigned hospital to find location of needed information to abstract (up to 15 different electronic systems)
  • Maintain a number of login information and confidentiality agreements for assigned hospitals
  • Abstract specific information required on HRR form/methodology
  • Review and maintain Portal Reports to maintain data quality
  • Contact Health Information Management Specialist (HIMS) with any questions regarding how to code abstracted information
  • Maintain privacy and confidentiality at all times
  • Ensure accurate transcription of information into database
  • Document completion of required chart review for each hospital for each time period
  • Work in compliance with Occupational Health and Safety/ Accessibility for Ontarians with Disabilities Act
Here is what you will need to be successful:
Education and Experience- Bachelor's degree or Diploma in Science, Arts, Humanities or Management- Successful completion of a recognized Health Information Management Program- Currently certified with the Canadian Health Information Management Association (CHIMA) and in good standing- Minimum of two (2) years of experience in related role or a combination of equivalent experience and education

Knowledge and Skills- Demonstrated attention to detail and data integrity- Demonstrated ability to work independently- Demonstrated ability to organize and prioritize work to meet quality and productivity standards and workload demands- Proficient in the use of Microsoft Office Applications, specifically Excel and Outlook- A valid G class driver's license and access to a reliable vehicle is required- Proven knowledge of various electronic health information systems required- Demonstrates a strong ability to extract information required for research
