QUALIFICATIONS:
  • Bachelor's degree in a healthcare/related field with active membership in a health or health related professional college
  • Nursing, Social Work, Physiotherapy and Occupational Therapy)
  • Must be a member in good standing of respective regulatory College (COTO, CNO, OCSWSSW)
  • Current license/registration in related field
  • Three years of hospital clinical experience in mental health and addictions setting.
  • Two years recent experience in case management, utilization review/management understanding of Alternate Level of Care (ALC,) patient care flow process and of community services
  • Experience in collaborative care within hospital and community setting
  • Demonstrated knowledge of medical terminology, ability to understand patient care plans, pathways and treatment plans.
  • Demonstrated knowledge of coordinating and leading discharge planning/destination meetings with the interdisciplinary care teams and patients and their families.
  • Evidence of effective organizational, interpersonal, communication, leadership, judgment and decisionmaking skills
  • Applies Niagara Health values and professional practice standards to patient/client care and service
  • Demonstrated commitment to patient/client centred care
  • Applies skills by providing, facilitating and promoting quality patient care within an interprofessional team
  • Demonstrated commitment to patient and family education and the ability to promote health and wellness
  • Integrates education, critical thinking, and relevant practical experience in problem solving
  • Proficiency in both official languages (English & French) will be considered an asset.
  • Consistent and regular attendance required.
  • Full vaccination in accordance with Niagara Health's Vaccination Policy
