6 FTE Refugee Health Integration Care Team Coordinator role (10 month contract)


The Refugee Health Integrated Care Team (RH ICT) project started as a test of change supported by the KW4 OHT.

The concept of this project is taking an integrated care team approach which will support non-team-based primary care physicians in KW4 to care for refugees and potentially other complex clients.

Several KW4 OHT member organizations are leading this initiative along with the Centre for Family Medicine, Carizon Family Services, Home and Community Care Support Services (HCCSS) Waterloo Wellington, Kitchener Downtown Community Health Centre/Sanctuary Refugee Health Clinic (KDCHC/SRHC) and Reception House.


The RH ICT team has multiple providers in different service locations (not co-located) which are all based within Waterloo Region.

This team currently includes one ICT Coordinator, three Newcomer System Navigators, one HCCSS Care Coordinator and access to on-demand interpretation services.

Together this team will support participating primary-care physicians by directly delivering and linking clients with primary care and community-based services.


This new ICT Coordinator will work together with the team by sourcing non-team based primary care providers within Waterloo Region and engaging in conversations to improve and enhance patient attachment opportunities.

This role will also work closely with the other ICT Coordinator to support transitioning patients from CFFM RHC and KDCHC/SRHC to primary care providers offices as permanent rostered patients.

This role will be working onsite at either KDCHC or CFFM RHC, along with visiting clinics in the community as needed.


We are looking for a creative and flexible professional who is comfortable working in and supporting the development of a new program and who wants to work in a team-based environment.

Preference will be given to individuals with second language skills and/or experience using interpretation services.


Qualifications and Experience:


  • Bachelors or Master degree in a relevant discipline e.g., social work, education or any other health profession
  • Strong experience in case management
  • Experience or strong interest in working with marginalized populations, patients with addictions, trauma histories and mental health diagnoses
  • Excellent problem solving and interpersonal skills
  • Excellent verbal and written communication skills
  • Demonstrated ability to work within teams
  • Demonstrated ability to work with ambiguity and mínimal supervision
  • Second language in any of Arabic, Somali, Tigrinya or Spanish preferred
  • Evidence of creative professional activity preferred
  • Vulnerable Sector Check required
