Highly innovative and fast-paced program seeking high energy, organized, flexible Diabetes Navigator.

The Diabetes Navigator would serve as a resource for patients and health care providers in the area of diabetes self-management assessment, education, and follow-up.

The main goal for the Diabetes Navigator is to ensure a smooth transition from pediatric to adult care and that the patient is able to use the tools and knowledge to independently manage his or her disease.

The individual in this role works collaboratively with other colleagues, leadership, physicians, and hospital services to assist in the development of an effective, patient-centered program that promotes optimal patient outcomes.

This position operates in the outpatient setting and works directly with patients/families one on one, in a classroom setting, and over the telephone.

The Diabetes Transition Navigator will provide evidence based comprehensive and effective transitional support for young adults with diabetes moving from pediatric to adult diabetes health care services in the Waterloo Wellington Regional Diabetes Program.

Referrals will be received through the Waterloo Wellington Regional Diabetes Referral Centre. The Diabetes Transition Navigator is accountable for the transition planning for adolescent diabetes patients.

The Diabetes Transition Navigator will serve as a patient advocate, liaison, and advisor/educator to assist patients and families in transition planning.

The Navigator will work as an integrated member of the health care team and in conjunction with partners within WW and the community.

The Diabetic Navigator's main areas of responsibility include: 1) Responsible for identifying appropriate education needs, goals and interventions with patients/families. 2) Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access and keep medical appointments, participate in recommended care plans, and receive appropriate health information about their diagnosis and treatment plan. 3) Responsible for addressing specific concerns as outlined for each patient in their initial assessment. Concerns are goal specific and would be included in the treatment plan for the patient. 4) Monitors and evaluates the effectiveness of these interventions and goal achievement, revising as necessary. 5) Identify best practices in diabetes self-management education, training and support. 6) Analyzes and evaluate a variety of program related data from multiple sources for the purpose of ensuring high quality, benchmark program initiatives and activities. 7) Conduct diabetes education inventory and evaluation. 8) Ensures that the cultural background of all patients are respected and honored. 9) Communicate effectively with colleagues, leadership, physicians, and community members. 10) Engages participants in learning with a positive attitude.


Qualifications:


  • Baccalaureate degree in a recognized health care discipline (Registered Nurse or Registered Dietician)
  • Master's Degree Preferred
  • Certified Dietician Educator Preferred
  • A comprehensive knowledge base in diabetes management is required.
  • 5 years of clinical practice is required.
  • 23 weeks of orientation to the hospital, department, staff, roles and responsibilities of position and close supervision/shadowing before practicing to full scope.
  • Basic understanding of health insurance requirements and benefit structures including knowledge of diabetic coverage issues.
  • Active member of the diabetes team and WW Regional Diabetes Network
  • Thoughtful and attentive listener
  • Good attendance record
