Position SummaryClinical Nurse Specialist, Advanced Practice Nursing Clinical Fellowship in Palliative Care
The Advanced Practice Nurse (APN) Clinical Fellowship in Palliative Care at North York General Hospital is a 6-month funded, interdisciplinary education program to train APNs who wish to become nursing leaders in the field of palliative care.
The curriculum is designed to build upon the core knowledge of master's level trained nurses and provides extensive training in the areas of pain and symptom management, end-of-life care, communication skills, grief and bereavement for both cancer and non-cancer illnesses.
After completion of this program the APN will be able to demonstrate application of effective knowledge, skills and attitudes to manage the palliative care needs of patients across different locations.
The APN fellow will provide care which recognizes the complex interplay of physical, psychological, social and spiritual needs according to the individual needs of patients with advanced illness and their families.
The APN fellow will also participate in education, quality improvement/ research as applicable and will complete a scholarly project.This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, learners, volunteers, and families.
On a practical level, you willMonthly learning objectives developed in collaboration with APN preceptors.
Regular meetings with the APN preceptors to discuss weekly reflections and progress.
A log of all patient's seen and learning points from each case.
A scholarly project developed in collaboration with APN preceptors with a final presentation to the Freeman. Centre for the Advancement of Palliative Care team.
There will be an evaluation at mid-term and at the end of the fellowship (3 & 6 months).
CLINICAL COMPONENTS OF THE FELLOWSHIP:
Acute hospital inpatient interdisciplinary palliative care service.
Medical Oncology clinic. Community/in-home palliative care service.
Ambulatory palliative care clinics for cancer and non-cancer illnesses.
Palliative Radiation Oncology off site.
Hospice placement off site.
Qualifications
Master degree in Nursing with aspirations to pursue a career in palliative care or a strong interest in the integration of palliative care into his/her current practice.
Current Basic Cardiac Life Support (BCLS) certification.
Demonstrated /dedication to the field of palliative care.
1-2 years' recent experience in oncology/palliative care nursing
Ability to use critical thinking, problem-solving and decision-making skills.
Ability to be self-directed in his/her own learning
Ability to collaborate effectively with all members of the interdisciplinary team and patients/families.
Excellent communication and interpersonal skills.
Demonstrated and ongoing commitment to professional development.
