Regional MINT Memory Clinics
The CFFM's mission is "To provide leadership and excellence in comprehensive patient-centred primary care through education, research, innovation and care provision in a collaborative, sustainable, inter-professional environment.
The Centre for Family Medicine's academic Family Health Team is comprised of six sites and is an integral part of the University of Waterloo Integrated Health Building as well as the McMaster Waterloo Regional Medical Campus.
MINT Memory Clinics
The Multi-specialty INterprofessional Team (MINT) Memory Clinic is an inter-professional approach to providing early diagnosis and treatment for persons living with memory loss.
Team members work with family physicians to provide comprehensive care for patients and family members dealing with dementia and other conditions involving memory loss.
Our goal is to improve access to high quality person-centered dementia care for older adults and their caregivers through team-based care provided in primary care.
Position Overview
Reporting to the Elder Health Manager and oversight with the Clinical Director, this position is a key in office presence supporting the flow of clinic and coordination for the Regional MINT Memory Clinic in Kitchener-Waterloo (KW).
Responsibilities
Regional Memory Clinic Coordination:
- In partnership with CFFM Memory Clinic clinical staff you will support, oversee memory clinic wait list and patient bookings.
- Support scheduling of all Regional Memory Clinics, including staffing in PSS.
- Respond to family physician inquiries and patient/family inquiries.
- Participate in clinic and program evaluation as required.
- Liaise with staff at Specialized Geriatric Services Clinical Intake to prioritize patient referrals and bookings.
Regional Memory Clinic Administration:
- Various administrative support include inputting patient referrals, preparing mail outs, making reminder calls, answering incoming calls from patients and physician offices, documenting in PSS, checking in patients, scanning.
- Faxing: appointment notices to patients, out going prescriptions, consult notes and tracking incoming medical imaging.
- Editing and faxing consult notes.
- Ensuring clinic days run smoothly
- Working closely with physicians and their schedules. Responding to physician inquiries.
- Communicating effectively with patients, care partners and other staff to provide a highquality care experience.
- Taking minutes for team meetings
- Assisting team members when needed
Requirements:
- 3+ years of administrative experience in a healthcare setting preferred.
- Experience working in interdisciplinary teams and liaising with clinicians, patients, families and other care givers
- Strong analytical and problemsolving skills
- Solid organizational skills including attention to detail and multitasking skills
- Ability to work well with multiple teams across multiple locations
- Ability to work with ambiguity and mínimal supervision
- Excellent written and verbal communication skills
- Experience working with individuals with memory loss an asset
- Compassionate verbal communication skills
