Niagara Health is located on the traditional territory of the Haudenosaunee and Anishinaabeg peoples. This territory is covered by the Upper Canada Treaties and is within the Dish with One Spoon Wampum agreement.

In the spirit of respect and friendship Niagara Health is embarking on a new quality improvement collaborative to better support Indigenous patients and their families.

Reporting to Manager, Indigenous Health Services and Reconciliation, and working collaboratively with the Patient Relations department and all members of Niagara Health.

the Indigenous Patient Relations Specialist will provide support for Indigenous patients and their families, by facilitating the coordination of access to services provided at Niagara Health and facilitate communication with the patient/family and all members of the care team in a culturally supportive manner.


RESPONSIBILITIES:
The role will have a focus on the following areas of service delivery:

  • Provide personalized support for Indigenous patients and their families accessing services at Niagara Health.
  • Maintain a high level of confidentiality and safety in support of a respectful, inclusive and compassionate care journey for Indigenous patients, their families and caregivers, and members of their care team.
  • Facilitate communication with the patient/family and all members of the care team to promote a culturally safe care environment and the delivery of highquality patient care.
  • Facilitate access to traditional healing practices including, cultural and spiritual practices for patients and families at their request.
  • Support the building of relationships between hospital, community partners and Indigenous communities based on mutual trust, respect, open communication, creative thinking, and cohesive team effort.
  • Foster an environment of cultural safety for all patients, caregivers, and their families;
  • Document patient stories where appropriate.
  • Provide support and consultation to healthcare providers where appropriate to facilitate access to care, services or resources for Indigenous patients, their families and caregivers;
  • Work collaboratively with other Indigenous Navigators across the care continuum to facilitate access and the coordination of care.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.

QUALIFICATIONS:


  • Five years of navigation, coordination, or other similar experience in healthcare or the social services sectors;
  • Lived experience (must selfidentify as Indigenous);
  • Strong knowledge of the history of Indigenous Peoples and the impact of intergenerational trauma;
  • Knowledge of local Indigenous organizations and community resources;
  • Knowledge and experience with traditional ceremonies and healing practices;
  • Ability to perform some culturally relevant ceremonies would be an asset;
  • Excellent communication skills (both written and oral);
  • Superior interpersonal, collaboration and facilitation skills;
  • Strong problemsolving abilities;
  • Demonstrated ability to work effectively in a multidisciplinary team environment;
  • Possession of a valid driver's license and access to a reliable vehicle.
  • Consistent and regular attendance required
  • Ability to speak an Indigenous language is an asset.
  • Vaccines (COVID19 and others) are a requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code.
