    Position Summary:

    Trillium Health Partners (THP) is excited to open an 18 bed Geriatric Unit at our Mississauga Hospital location and is recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and interprofessional skills. As a member of the Geriatric Medicine Interprofessional team you will have the opportunity to provide patient-centred care to the frail older adult with the goal to prevent cognitive and functional decline during admission and participate in plans for same discharge home. This role will report to the Clinical Manager of Seniors' Services.

    Responsibilities:

  • Responsible and accountable for decisions and actions in the provision and management of Nursing care for patients in accordance with the standards of nursing practice and best practice guidelines
  • Assess and classify physical and emotional condition of patient through reports from staff and interviews with patient and family
  • Responsible for the provision of nursing care, teaching and coaching to patients and their families
  • Collaborate with the interprofessional team in the provision of care and care planning.
  • Key Qualifications:

  • Current registration with the College of Nurses of Ontario
  • In good standing with the College of Nurses of Ontario
  • Current BCLS Certificate
  • Reliable and flexible individual to work as part of an interprofessional team
  • Experience in Geriatric Medicine is an asset
  • Ability to think critically and effectively in order to prioritize patient care needs
  • Able to work in a fast paced environment within highly collaborative model of care
  • Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
  • Positive attendance and work record essential
  • Candidates are selected on the basis of their skill, ability, experience and qualifications.
