    SOCIAL WORKER (MSWI)
    • Position Type: Permanent Full-time
    • Shift Type: Day, No weekends (subject to change)
    • Hourly Pay Rate: $44.22- $57.35Bi-Weekly Hours: 70 Hours, 7hrs/shift

    The Apotex Centre, Jewish Home for the Aged, is a 472-bed long-term care facility providing various residential and specialized programs. Unique home-like settings offer flexible schedules for programs, meals, and personal care based on residents' individual needs and preferences. Residents at Apotex require aid to maintain their lifestyles and achieve their daily objectives.
    Responsibilities include but are not limited to:
    • Conduct tours of Apotex for potential residents and families
    • Collaborate with Ontario Health and care team for admissions
    • Manage resident admissions process and assessments
    • Deliver psychosocial support and counseling for residents and families
    • Implement resident-centered care plans alongside the team
    • Arrange care conferences and support resident advisory council
    • Offer guidance on psychosocial issues to the team
    • Provide crisis intervention and act as a communication link
    • Establish community connections with relevant agencies
    • Engage in teaching and student supervision activities
    Qualifications include but are not limited to:
    • Masters of Social Work degree
    • Active registration with the Ontario College of Social Workers and Social Service Workers ('OCSWSSW')
    • Background in Minimum Data Set ('MDS') assessment preferred
    • Strong interpersonal, communication, and problem-solving skills
    • Familiarity with Meditech, Microsoft Word, and Excel
    • Knowledge of Jewish culture and additional languages like Yiddish or Hebrew beneficial
