The Opportunity
This position is responsible for providing client care through focused assessment, treatment and dischargeplanning on our rehabilitation and recovery units, and contributing to our resident's quality of life through assessment ofappropriate interventions to maximize safe, functional independence on our continuing care units. Other duties may includeproviding clinical supervision to support staff and students. Responsible for other related duties as required.
Program summary:
Comprehensive Community Care (C3) Program – a unique, long-term maintenance and support program for seniors who live inCalgary. Integrated and coordinated care is provided by a 24-hour health care team through a medical health clinic, dayprogram, home support, transportation, access to respite beds and emergency response telephone system.
