    This role is Casual and located in Norfolk, ON.

    What The Role Involves:

    Developing effective Care Plans for Community Health Services independently or in collaboration with Home and Community Care Support Services (HCCSS).

    Providing exceptional nursing services to patients in various settings, including home, clinic, retirement home, or school

    Communicating with patients, family members, and interdisciplinary care team

    Performing clinical duties as required


    What You Bring:
    • Registered Nurse (RN) or Registered Practical Nurse (RPN) qualification
    • Current College of Nurses of Ontario (CNO) registration
    • Passion for patient care
    • Excellent interpersonal skills
    • Valid driver's license and vehicle
    • Strong English communication skills
    • CPR Certification
    • Current immunization record
    • Vulnerable sector check and clear background check

    In addition to home-based health care, CarePartners supports the community through clinics, transitional care units, and relief services in retirement homes and shared care settings.

    Our Community Nursing Services outreach program has been providing medical care and clinics in underserved regions since 2009.
