Detailed Overview

The Community Health Nurse (CHN) - Registered Nurse role at Fraser Health involves independent work in the community setting. Collaborating with an interprofessional team, the role focuses on managing client caseloads, emphasizing health promotion, maintenance, and restoration through various interventions.

Responsibilities

Establish a therapeutic relationship with clients and ensure autonomy in decision-making.
Provide individualized assessments, care planning, and interventions for clients with complex chronic conditions.
Develop comprehensive care plans and facilitate transitions between healthcare providers and services.
Offer direct client care, education, and support using evidence-based best practices.
Coordinate and monitor client care through care conferencing and goal setting.
Provide health counseling, education, and wellness promotion to clients.
Maintain accurate clinical records and stay updated with current standards of practice.
Participate in quality improvement activities and contribute to policy development.
Support the orientation and education of nursing staff and students.
Perform other relevant duties as assigned.

Qualifications

Education and Experience

Requirements include current registration as a Registered Nurse, one year of recent clinical experience, and a valid BC Driver's license with access to a vehicle.

Ability to problem-solve, prioritize, and work independently or within a team.
