POSITION SUMMARY:
The Dietitian isresponsible for the provision of dietetic and nutritional services to Home andCommunity Care patients in order to optimize nutritional intake, promoteoverall health and well-being, and individualize food plans to meet specificnutritional, dietary, and/or functional needs.
QUALIFICATIONS
· Registered Dietitianin good standing with the College of Dietitians of Ontario(CDO);
· Relevant experienceproviding nutritional and dietetic services in a community/health careenvironment;
· Comprehensiveknowledge of dietetic services, practice methods, professional skills,nutritional principles and dietary strategies;
· Strong interpersonaland collaboration skills to manage service delivery in conjunction with casemanagers, patients, their family members/caregivers, and other health careprofessionals;
· Good understanding ofthe roles of other health care professionals and complementary therapies;
· Knowledge of fundingagencies available to assist patients in accessing needed support services andcommunity resources andother communityresources, treatment centres, and organizations;
· Practical knowledgeand understanding of pertinent legislation;
· Comprehensiveknowledge of the standards of practice and professional guidelines set forth byCDO;
· Ability to prioritizeprofessional duties and manage caseload in a time efficient manner;
· Ability to accuratelycomplete required documentation, reports, forms, and recommendations;
· Effective listening,observation, and facilitation skills;
· Ability to communicateinformation effectively through a variety of means including meetings, reports,letters, and presentations;
· Working knowledge ofcomputer software (email, internet) and Microsoft Office applications (Word,Excel);
· Advanced oral andwritten proficiency in English is essential.
