- Provides comprehensive assessments and client care plans for elderly clients in the community
- Maintains responsibility for establishing a client care plan based on bio-psychosocial and spiritual needs assessment and integrating other health professionals' recommendations
- Plans for, delivers and evaluates mental health interventions and treatments which fall within their scope of practice
- Documents care provided and responses observed
- Facilitates communications between care providers in the community
- The clinician will further assist with triage and intake
- The clinician will assist with capacity building and knowledge exchange. They will work to model best practices of care.
- Completion of a Master's degree program in Social Work
- Current and valid registration with the Ontario College of Social Workers and Social Service Workers ('OCSWSSW')
- Minimum three (3) years related experience in geriatrics, psychiatry, community-based seniors care and/or case management behavioural management
- Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
- Demonstrated excellent clinical-reasoning, critical thinking and problem-solving skills
- Demonstrated skill in counselling, psychoeducation and support
- Demonstrated knowledge about psychiatric conditions and illnesses
- Access to transportation (e.g. current driver's license and car) required
- Excellent inter-personal, communication, problem-solving and critical thinking skills
- Basic computer skills – Meditech, Microsoft Word, Excel and Outlook preferred

Job Summary:
Ambulatory Outreach has a part-time opportunity for a Social Worker to join the interdisciplinary Geriatric Psychiatry Community Service. This position will work collaboratively with all three outreach teams: Geriatric Psychiatry Community Service, Integrated Community Care Team, and Interprofessional Primary Care Team to provide assessment and treatment to older adults living in the community.
