Reporting to the Manager of the Concurrent Outpatient Medical Psychosocial Addiction Support Service (COMPASS), and working collaboratively with the COMPASS practice leadership group, the Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic's interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH's vision and strategic directions. The Team Lead will assume responsibility in supporting the intake coordination of the service. They will support the communication & liaison with internal and external stakeholders, scheduling of intake assessments and client appointments in direct collaboration with our Information Specialist team. As part of the Team Lead's responsibility, the successful candidate will contribute to treatment planning and case management, as well as support the coordination of client/patient intake, discharges, and transfers of care. The successful candidate will facilitate urgent service delivery and support our Rapid Access Addiction Medicine Clinic, Fit-In Clinic, and Day Detox to ensure a high standard of quality care and safety. The Team Lead will coordinate with the Manager regarding staff schedules to ensure appropriate coverage of the clinic. In collaboration with practice leads you will link new staff with appropriate clinic resources and facilitate orientation to the clinic, ensuring they become familiar with the clinic's procedures/processes. You will coordinate and/or assign, in collaboration with clinical supervisors/preceptors, an appropriate CAMH staff to assist students of all disciplines in providing a safe and respectful learning environment. The Team Lead will collaborate with the manager regarding staff participation in off-unit meetings/ trainings, educational events. You will collaborate with the Manager on monthly reports and the collection of other relevant data sets. You will support a healthy workplace that embraces diversity, promotes positive team dynamics and reinforces collaborative practice, as well as complies with all applicable standards and required organizational practices. You will contribute to the development, implementation and evaluation of quality improvement initiatives and promoting best-practices, all of which advance excellence in nursing and client/patient care. The Team Lead will also be responsible for conducting assessments and providing client-centered psychiatric, addictions and general nursing care. You will practice in accordance with CNO standards, with CAMH Policies and the CAMH Practice Model, and the Canadian Standards of Psychiatric and Mental Health Nursing Practice (CFMHN).
The successful candidate must be a member in good standing with the College of Nurses of Ontario. A current B.C.L.S. (CPR) certificate is also required. Preference will be given to candidates who possess or are currently pursuing a BScN and CNA certification in Psychiatric/Mental Health Nursing. At least two years (or 3900 hours) of clinical experience within the addictions and mental health field is required, as is experience in addiction medicine. Previous experience in case management and clinical assessment is required as is the knowledge and understanding to support and triage rapid access referrals and urgent care needs related to addiction medicine treatment. The successful candidate will possess proficiency and leadership in problem-solving, decision-making, conflict resolution, negotiation, communication, delegation, critical thinking, team building and interpersonal skills. The successful candidate will possess the ability to think at a program-level and participate in unit-level as well as organization-wide change initiatives. The individual will have the ability to communicate effectively with the team, including service leadership, both verbally and in writing; knowledge of the roles of interdisciplinary team members, their professional responsibilities and areas of expertise is essential. Demonstrated expertise in maintaining a culture of safety for staff, clients/patient, families and visitors according to CAMH and regulatory standards is critical. Bilingualism (French/English) or proficiency in a second language would be an asset.
