    We are Home and Community Care Support Services, ready to serve everyperson in Ontario.

    We partner with patients and caregivers, primarycare providers, hospitals, long-term care and retirement homes, serviceproviders and Ontario Health Teams, to deliver responsive, accessible,integrated, patient-centred care.

    Are you an experienced registered nurse, physiotherapist, occupationaltherapist or social worker seeking a rewarding career that cares for others, ina professional practice that cares for you? As a CareCoordinator , you will assess and determine patient care needs andeligibility, provide access and referrals to community services, and engagewith patients, caregivers and other health care practitioners.

    Whether you work in ouroffice, in the community, or a health care facility – you will play a lead rolein providing connected, accessible, patient-centred care – and be supported byour collaborative team that includes over 8,000 regulated health care and otherprofessionals.

    A sa valued team member, your mission will be to help our patients be healthier at home, while you benefitfrom our supports for professional growth, personal wellness and work-life balance.

    In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

    • Coordinate and monitor care plan delivery
    • Establish a helping relationship with patients and their families
    • Membership, in good standing, with theapplicable regulatory body:
    o Ontario College of Social Workers and SocialService Workers
    Recentexperience in community health or a related field
    Knowledge ofthe health care delivery system and community resources
    Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
    We have a mandatory COVID-19vaccination policy. As a condition of employment, all employees arerequired to submit proof of COVID-19 vaccination status prior to startdate.
    multicultural, homeless, palliative,acquired brain injury (ABI), mental health, geriatrics, pediatrics

    • Casemanagement experience or recent related community experience
    • Abilityto speak French or another second language
