    Description

    Provides social work services to residents and patients family along the continuum of care from admission to discharge. Acts in an advocacy role as appropriate.
    orientation adjustment programs, crisis problem solving, conflict resolution / mediation, bereavement counseling, palliative care etc.
    In collaboration with the Director of Care, facilitates tours of facility
    In collaboration with the Operations/Office Manager, assists with admissions and adjustment issues.
    Participates in providing in-service programs for facility staff, residents, and patients, and families as needed.
    Liaises with educational institutions and provides supervision to social work students on clinical placement.
    Participates on committees and teams related to resident care and operation of the facility as required (i.e. management team, customer service, palliative care, ethics etc.).

    Liaises and consults with area hospitals, community social service organizations and government programs in relation to resident care issues and departmental activities.

    Master's (preferred) or Bachelor's degree in Social Work from an accredited University;
    Previous experience in a long-term care setting an asset
