    Description

    Job Description

    • Provide patient-focused, safe, and holistic nursing care while collaborating with our specialized palliative care team.
    • Ensure nursing care is based on current best practices to achieve successful outcomes.
    • Continually assess, anticipate, identify, and respond to patients' changing needs.
    • Travel between patients' homes within a specific geographical region.

    Qualifications:

    • Minimum two-year nursing experience (Community Nursing experience is ideal).
    • Registered and in good standing with the College of Nurses of Ontario (CNO).
    • Experience working with palliative care patients.
    • Fundamentals of palliative care, LEAP, and/or CAPSE courses are an asset.
    • Ability to work independently and manage your own caseload.
    • Proficiency in English, both verbal and written required.
    • Valid Ontario driver's license and reliable vehicle for travel.
    • Excellent patient assessment and teaching skills.
    • Problem-solving and critical thinking skills.
    • Physical ability to perform day-to-day duties.
    • Basic proficiency in Word, Excel, and software.
