Utilizing a primary model of care the LPN will provide resident care which includes assessing developing care plans; monitoring; participating in client focused teams with other healthcare providers.
Advocating for clients including communicating client needs to healthcare stakeholders; Educating and training resident's and families on health care issues; raising awareness of community services.
Supervision and monitoring including teaching, delegated acts to health care providers (professional and support staff); Monitoring peers and preceptor ship (new LPN's to students).
Information management including documentation, producing initial, interim, and, if needed, discharge reports. This also includes Resident Care Plans. Currently licensed with the College of Licensed Practical Nurses of Manitoba. Knowledgeable in the principles and philosophy of long term care. Demonstrates concern and interest in providing quality care to residents. Experience 1 year in a Long Term Care setting, 1 year clinical, supervisory experience.
