    Your Opportunity:

    Within Cancer Care Alberta (CCA), Supportive Care services are provided by a team of health care professionals dedicated to enhancing the quality of life of those affected by cancer by providing support from the time of cancer diagnosis, through treatment and beyond. The team of professionals that work within CCA are leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care, all on a foundation of world-class research.The Social Work team at the Tom Baker Cancer Centre needs a temporary full-time Social Worker II (SWII) to join the team. The SWII plays a crucial role in empowering patients and their caregivers to address the social determinants of health. If you are a patient-centered collaborator with social work experience and a passion for working in cancer care, this job is for you.Under general supervision from the Acting Lead of Supportive Care, South, the SWII participates in referral triage, screening, resource and supportive counselling, psychoeducational groups, quality improvement initiatives, research, and service evaluation. The SWII also engages in continuous professional development and strives to work to their full scope of practice.Along with the team, the SWII supports orientation and mentorship of new employees and students. Effective time management and excellent written and verbal communication skills are essential in this role. The applicant will be skilled at developing collaborative partnerships with community programs to help facilitate access to services for ongoing needs. The caseload ad hours of work for all positions within Psychosocial Resources may be subject to change based on operational needs.

    Description:

    As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

    Required Qualifications:

    Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

    Additional Required Qualifications: Preferred Qualifications:

    Experience working within a chronic disease management framework with adult populations in an urban outpatient setting within the last 5 years. Experience working in an adult acute care setting within the last 5 years.Experience facilitating groups.Experience providing education to patients and families about cancer.Experience supporting clients through grief and loss. Experience developing partnerships / collaborations within and across organizations.
