We are seeking an Occupational Therapist for our newly established MGH2Home Program – FT – Salary Employee
What will be your role?
This is a unique Occupational Therapist position supporting East Toronto Health Partners' (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program. This short-term transitional program (up to 90 days) involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as "one team", along with the client and designated family, to address their complex health and social needs in the development of a single "one team" care plan.
As an active member of a multidisciplinary team, OTs will provide consultation, assessment and planned interventions for diverse client groups in the community discharged from MGH2Home program. The Occupational Therapist works within a continuous quality improvement framework, which reflects VHA's core beliefs and mission of services as well as promoting and maintaining good public relations with clients, customers and VHA staff, volunteers, and associates.
There is a $2,000 Sign-On Bonus for full-time occupational therapists only.
