    Clinical Coordinator Hereditary Kidney Disease Clinic
    Contract

    The Hereditary Kidney Disease clinic is a provincial resource designed to improve the healthcare of patients with these rare and complex kidney conditions.

    The Nursing Clinical Coordinator in the Hereditary Kidney Disease Clinic, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and referring physicians in these kidney conditions with the ultimate goal of preventing kidney disease progression.

    As a member of the Ambulatory Clinic (HKD) team, the responsibilities of the Clinical Coordinator, include: utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities, the coordination of patient care across the continuum and continuity in patient care management; participating actively in Quality Improvement initiatives; participating in academic and research activities/projects; performing administrative responsibilities; Bachelors of Science Degree in Nursing
    Certificate of competency (licensure) from the College of Nurses of Ontario
    Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand

    Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
