    What you will do:

    Program Philosophy

  • Demonstrate a deep understanding of the ACT Model, Harm Reduction, Trauma Informed Care, Mental Health Act and Community Treatment Orders.
  • Adhere to program policies and procedures to ensure the provision of high-quality care and support to our clients.
  • Administrative

  • Conduct monthly evidence-based assessment tools with program participants and accurately entering data.
  • Develop, implement, and update participant-centered service plan goals and safety planning in collaboration with participants and team members.
  • Complete all required documentation with precision and in compliance with agency policies, procedures, and professional college requirements.
  • Collect and enter data accurately to maintain records and track progress.
  • Assist with completing intake procedures for new program participants.
  • Participant Care

  • Provide single clinician assertive outreach services to individuals with acute, severe, and persistent mental health and substance use disorders.
  • Support program participants by connecting them to appropriate community services and facilitating referrals as needed.
  • Assist participants in developing and improving daily living skills such as cooking, cleaning, financial literacy, and grocery shopping, both in their suites and community settings.
  • Provide transportation support to external appointments and assist with activities of daily living as required.
  • Advocate for the rights of program participants and assist them with systems navigation, including justice, financial, and healthcare systems.
  • Aid participants in obtaining and maintaining income supports and provide medication assistance as required.
  • Demonstrate strong crisis support and de-escalation skills, along with a commitment to innovation and flexibility in your approach.
  • Assist participants with various needs including housing, housing location, entitlements, employment, education, and community integration.
  • Collaborate closely with partnering agencies to ensure effective participant service planning and goal achievement.
  • Team

  • Demonstrate exceptional written and oral communication skills, with a focus on active listening.
  • Participate in daily team meetings and case planning sessions.
  • Take initiative and work effectively as a team member, providing support to colleagues as needed.
  • Exhibit a high degree of professionalism and collaborate effectively with partner agencies.
  • Maintain a welcoming demeanor and provide ongoing feedback to promote a positive workplace environment, foster strong team cohesion, and ensure exceptional program delivery.

  • Facilitate group programming for participants and staff relevant to expertise.
  • What you bring:

  • Within one year of completing a Bachelor's degree or diploma in a relevant field is preferred
  • Minimum two years of experience in a social service setting
  • Strong knowledge base of local resources for housing, mental health, addiction, community support, finances, etc.
  • Comfort meeting individually with participants in the community, in a broad variety of settings, with a safety-first mindset
  • Knowledge of evidence-based interventions (e.g., motivational interviewing, de-escalation skills, crisis intervention, etc.)
  • Exceptional communication, organization, advocacy, teaching, and rapport-building skills
  • Commitment to contributing positively to team culture
  • Openness to a wide scope of practice with a commitment to innovation and flexibility
  • Strong initiative in times of limited supervision
  • Intermediate First Aid - Level C CPR within 30 days of hire date, certification must be valid for a minimum of 6 months upon hire
  • Class 5 driver's license and personal vehicle for business use, clean drivers abstract, plus $2 million liability coverage are required
  • A satisfactory vulnerable sector criminal record clearance is required
  • Knowledge of the United Nations Declaration on the Rights of Indigenous People (UNDRIP) and the Truth and Reconciliation Commissions (TRC) 94 Calls to Action is considered an asset
  • Willingness to work within anti-racist and anti-oppressive frameworks and actively engage in ongoing learning about these frameworks
  • Working environment:

  • Predominately outreach: single staff visits in program participant homes
  • Casual employment
  • Standard hours 8:30-4:30, Monday – Friday (with flexibility in crisis situations)
  • Continuous extended periods of sitting, standing, and walking
  • Frequent driving as part of work duties
  • Occasional climbing of stairs throughout shift
  • Occasional time outside in all weather conditions
  • Working Conditions:

    This position requires frontline staff, in some cases, to be exposed to emotionally disturbing events, such as medical distress, threatened/actual serious injury, violence, abuse, and death. There may also be exposure to situations where clients may be experiencing fear, terror, hopelessness, rage, self-harm and conflict. It is understood and accepted that this position will be working in distress/high-stress situations, which may include emergency response where resiliency, stress tolerance, and the ability to maintain composure is imperative. This position may also be exposed to bodily fluids. The health and safety - both physical and psychological- of our employees is a priority.
