You will co-ordinate your services with the other staff in the Clinic, which may require day and evenings as well as potential travel to our satellite sites. In this role, you will, in collaboration with the interdisciplinary health care team, be responsible for comprehensive assessment, care planning and education for patients and families with Diabetes. Current certificate of registration with the College of Nurses of Ontario; BScN Preferred. A minimum of two years' recent experience in a diabetes education/diabetes management setting; Certified Diabetes Educator designation required. Knowledge of the South Asian & South East Asian culture and ability to speak in a South Asian and South East Asian language is an asset. Certified Pump Trainer designation is an asset; Excellent knowledge of continuous glucose monitor and insulin pump management principles is preferred. Computer skills are essential.
